  • Property - Amount Subject
  • Special Property form enhancements (in most states) - Various Limits
  • Equipment Breakdown    - Amount Subject
  • General Liability – $1M per occurrence / $2M aggregate 
  • Inland Marine    - Amount Subject
  • Crime - Amount Subject
  • Commercial Auto – $1M combined single limit
  • Commercial Umbrella - $5M limit
  • Stop Gap Liability – $1M limit
  • Workers' Compensation    (available through another carrier) 

Submission Requirements:

  • ACORD Applications
  • Program administrator’s  Supplemental Questionnaire
  • Metal Finishing Supplemental Questionnaire if applicable
  • Foundries Supplemental Questionnaire if applicable
  • Prior Premium and Loss Information for all lines (3 years minimum)
  • Brochures or Product Descriptions 

Broker Requirements:
Current appointment with the program administrator or, if not, complete the following items:

  • Broker Agreement
  • IRS W-9
  • Copy of E&O Certificate or Declaration Page
  • Copy of the agency’s license per location
  • Agency Profile Document
  • If multiple locations, include a list and confirm ALL locations are covered under the operation’s Errors and Omissions policy
