    • RV Park or Campgrounds (unless incidental to the overall resort operations)
    • Golf or Country Clubs that do not have onsite lodging facilities

  • Automobile (except MA & HI), can only be written in conjunction with the Package policy
  • Crime – up to $250k limit

Submission Requirements:

  • Completed ACORD Applications
  • 5 years of currently valued loss runs with details on any claims over $10,000
  • Full description of operations including those of additional named insureds
  • Expiring and target premiums
  • Access the appropriate program specific  supplemental forms

Broker Requirements:

Current Appointment with the program administrator or, if not, complete the following items:

  • Broker Agreement
  • IRS W-9
  • Copy of E and O Certificate or Declaration Page
  • Copy of the agency’s license per location
  • Agency Profile Document
  • If multiple locations, include a list and confirm ALL locations are covered under the operation’s Errors and Omissions policy
